Office Information: About Your Visit
 Insurance Information
We participate in most major insurance plans. Be prepared to provide your insurance card to the receptionist at the time of your visit. If your insurance plan requires referrals, please request one from your primary care physician prior to your scheduled appointment in our office.
Appointments
Please call the office to make an appointment in advance. If you are unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery.
Fees & Payments
We make every effort to respect the cost of your medical care. Therefore, to keep your costs down, we request payment for all office services at the time they are rendered. If needed, special arrangements can be made prior to your visit. We accept cash, checks, Mastercard, and Visa for your convenience. If we are a participating provider with your insurance company, we will bill them. We will help in anyway we can to assist you in handling claims.

Prescriptions & Renewals
Requests for medication renewals are handled during regular office hours, Monday-Friday, 8:30am-5pm. Renewal requests at other times will be addressed the next business day. There is a prescription refill line on our office phone.
